Resource Coordinator Needed!

DESCRIPTION: 

The Resource Coordinator is a vital administrative team member responsible for the efficient organization and maintenance of resources within our computer system database, ensuring that data is accurately categorized, facilitating seamless retrieval. Beyond internal responsibilities, the Resource Coordinator actively engages with external businesses, establishing and nurturing partnerships for resource exchange. The Resource Coordinator plays a crucial role in the community by facilitating warm handoffs and ensuring resources are transferred smoothly for the benefit of all. Preferred candidates will have prior experience in resource management or a related role, bringing technical proficiency and collaborative expertise to the team.

Care Coordinator Needed!

DESCRIPTION: 

The Care Coordinator will play a critical role in assessing participant needs, developing personalized care plans, and connecting individuals with appropriate resources and services. This position requires a compassionate and organized individual who can collaborate with various service providers, monitor client progress, and advocate for client needs..


SKILLS NEEDED:

  • Associate’s degree in a related field.

  • Minimum of 2 years of experience in case management or a related role.

  • Strong understanding of community resources and social services.

  • Excellent communication, interpersonal, and organizational skills.

  • Ability to work independently and as part of a team.

  • Compassionate and empathetic approach to client care.

  • Proficiency in Microsoft Office and client management software.

  • Bilingual (Spanish/English) preferred but not required.
